The Public Health Nurse (PHN) delivers comprehensive nursing services that promote health and well-being for residents of Marion County across various settings, including homes, schools, workplaces, clinics, and community sites. The PHN conducts assessment, education, case management, care coordination, immunization, communicable disease services (including TB), and surveillance activities; documents care in the electronic medical record (EMR); and collaborates with interdisciplinary partners to advance equitable health outcomes. The PHN represents Community-Based Care and works across multiple MCPHD departments to assure continuity of care for diverse populations. This position reports to the Northeast District Health Office.
Your day blends field and clinic work. You will review referrals, conduct home visits with clients, administer immunizations in clinic, and provide TB case management services (DOT/DOPT coordination). You will document in the EMR, coordinate services with community partners, participate in outreach events, and respond to urgent public health needs. Throughout the day, you communicate clearly with families and multidisciplinary teams, uphold HIPAA, and advance health equity.

Protect and maintain confidentiality of client/employee information; secure records when transporting to other settings or home visits. Complete National Incident Management System (NIMS) IS-100 and IS-700. In a public health emergency, availability for weekends or alternate schedules is required.
Qualifications
Required Education Bachelor's degree in nursing (BSN) from an NLN-accredited program.
Required Experience 10 years of prior nursing experience Typing and computer proficiency (≥30 WPM) and EMR documentation experience
Preferred Experience Public or community health experience
Licenses/Certifications Required Current Indiana Registered Nurse (RN) license Current CPR certification
Knowledge, Skills & Abilities Clinical & Public Health Practice Skilled in independent nursing assessment, triage, care planning, motivational interviewing, and culturally responsive education. Immunization practice (vaccine administration, storage/handling), TB control (case management, DOT/DOPT), communicable disease basics, maternal/child health. Equity, Communication & Partnerships Demonstrated commitment to health equity; ability to communicate effectively across languages/cultures and build trust with diverse communities. Professional collaboration with agency/community partners; diplomacy and strong customer service. Regulatory & Documentation HIPAA, confidentiality, and informed consent; accurate, timely EMR documentation; data collection for program monitoring and reporting. Professional Judgment & Organization Prioritization, problem-solving, and sound clinical judgment in dynamic field/clinic settings; ability to function independently and within teams.
Working Environment Driver/Travel: Valid Indiana Driver's License, personal vehicle, and current auto insurance required for community/home visits. Physical demands: Sitting for interviews/EMR entry; standing/walking between community sites; stair climbing; frequent hand use; occasional lifting/carrying up to ~20 lbs. Exposure: Potential exposure to substandard/unsanitary conditions, allergens (pets, dust, smoke), communicable diseases, blood/body fluids. PE/Controls: N-95 use when clients are potentially infectious; adherence to standard and transmission-based precautions. Scheduling: May require occasional evenings/weekends; emergency response and alternative schedules during public health events.
